C/S Behavioral Health Case Manager III

Job Description

The BHCM III position at DPC on the civil side and forensic unit performs a fundamental role in the discipline management, recovery mission and compliance with inspection and audit criteria. The BHCM III will interview clients and family members to obtain detailed social history. They will connect and collaborate with providers to secure vital community supports, including but not limited to case management and housing. Other responsibilities include admissions tasks such as social assessments, facilitating groups, developing and implementing treatment plan goals/interventions and tasks to meet the needs of the hospital which may require occasionally working evenings and weekends.

JOB REQUIREMENTS for Behavioral Health Case Manager III
Applicants must have education, training and/or experience demonstrating competence in each of the following areas:

  1. Possession of a Bachelor's degree or higher in Behavioral or Social Science or related field.
  2. Three years' experience in case management which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual's human service needs.
  3. Three years' experience in providing clinical treatment, counseling or determining eligibility for services or benefits as part of a treatment team.
  4. Six months' experience in interpreting laws, rules, regulations, standards, policies, and procedures.
  5. Six months' experience in narrative report writing.
