Job Description
The Behavioral Health Case Manager III (BHCMIII) position sits in the Child and Family Care Coordination Unit which works with children and adolescents with serious behavioral health conditions and their families to help families access treatment and other services/resources that help the child/adolescent achieve success at home, school and in their community. The BHCMIII works directly with providers to monitor progress in treatment and works with community partners including child welfare, juvenile justice, schools and the Courts to help families navigate those systems. The BHCMIII works with a licensed clinician to support new staff through shadowing and other opportunities and also completes chart reviews for team members to ensure alignment with unit standards.To view the entire job announcement and apply, Click Here.
JOB REQUIREMENTS for Behavioral Health Case Manager III
Applicants must have education, training and/or experience demonstrating competence in each of the following areas:
- Possession of a Bachelor's degree or higher in Behavioral or Social Science or related field.
- Three years' experience in case management which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual's human service needs.
- Three years' experience in providing clinical treatment, counseling or determining eligibility for services or benefits as part of a treatment team.
- Six months' experience in interpreting laws, rules, regulations, standards, policies, and procedures.
- Six months' experience in narrative report writing.
