Team Lead

Program: ACT Team

Location: New Castle County

Status: Full-Time - 40 hours

Schedule: M-F 8a-4p

ABOUT THE ROLE

The ACT Team Leader is a clinician with administrative and clinical responsibility to manage a multidisciplinary team of professionals and para-professionals whose ultimate goal is to encourage and facilitate recovery with up to 125 individuals living with and recovering from a severe and persistent mental illness. The Team Leader will understand multiple evidenced based practices, including Assertive Community Treatment, Integrated Dual Disorders Treatment, Illness Management and Recovery, Evidenced Based Supported Employment (Individual Placement and Support), Pathways to Housing, Trauma Informed Care and Family Psycho-Education.

RESPONSIBILITIES

    • Lead and inspire a multidisciplinary team to deliver trauma-informed, recovery-focused services.
    • Ensure effective delivery of ACT services in compliance with policies, regulations, and ethical standards.
    • Monitor program outcomes and implement corrective actions to improve quality.
    • Supervise all clinical and administrative activities of staff.
    • Oversee accurate and timely documentation in the Electronic Health Record (EHR).
    • Ensure program fidelity and compliance with billing and contractual targets.
    • Coordinate and attend staff and treatment planning meetings.
    • Provide clinical and administrative supervision, including staffing schedules and crisis response availability.
    • Promote evidence-based practices such as motivational enhancement therapy and relapse prevention.
    • Collaborate with internal and external partners to improve client access to care.
    • Develop and maintain safe, affordable housing options for clients.
    • Oversee coordination of entitlements, vocational, housing, and medical services.
    • Maintain the physical appeal of the program site.
    • Foster group decision-making and accountability within the team.
    • Perform other duties as assigned.

REQUIREMENTS

    • Education: Doctoral or Master’s degree in psychology, counseling, social work, nursing, rehabilitation, or related field; or RN with mental health nursing certification.
    • Experience: Minimum of 3 years clinical/administrative experience with adults with SPMI and co-occurring substance use disorders.
    • Certifications: CPR and First Aid.
    • Licensure: Valid driver’s license with at least 3 years of clean driving history.
    • Background: Must pass a criminal background check.
    • Skills:
    • Critical thinking and organizational skills
    • Strong communication and interpersonal abilities
    • Ability to work with diverse populations and route concerns appropriately
    • Familiarity with confidentiality regulations (PM 46, 42CFR Part 2, 45 CFR)
    • Computer literacy in MS Office and EHR systems
    • Physical Requirements: Ability to perform tasks such as sitting, standing, bending, lifting, climbing stairs, and driving.

ADDITIONAL QUALIFICATIONS

    • Computer knowledge of MS Office, and other HR applications is a plus, not required.
    • Possess strong verbal and written communication skills.
    • Must be proactive, organized, and possess great follow-up skills.
    • Must have the skill/ability to collaborate, partner with, and provide exceptional customer support to a variety of affiliate partners within the organization.
    • Three- year Motor Vehicle Record and valid Delaware driver’s license
