SUMMARY DESCRIPTION
The primary focus of this position is to provide high-quality, efficient, and safe personal assistance, and non-medically skilled service to customers.
ESSENTIAL DUTIES & RESPONSIBILITIES
This listing is representative of the duties and responsibilities of the position, but is not to be construed as all inclusive:
- Provide services to customers by following the Service Plan which may include the following types of care: personal care to include bathing, skin care, toileting, dressing, feeding, and shaving. Physical care to include assisting with transfers and ambulation. Supportive care to include preparing meals, light housekeeping, laundry, changing linens, shopping, local transportation, and companionship.
- Successfully complete all required training requirements including recurrent and ongoing requirements. Must successfully complete knowledge and competency tests at least annually demonstrating understanding and ability to perform Personal Assistance Services duties.
- Complete assigned work, as per the Service Plan, accurately and within the appropriate time.
- Document on timesheet, Direct Care Daily Activity Log, and observation report form as indicated. Report problems or significant changes in customer condition to Program Director immediately.
- Complete the timesheet accurately, completely, and within the required time.
- Attend all in-service training sessions and meetings as required.
- Accept fill-in assignments as schedule allows and per request of the Program Director.
- Respond to calls from CHEER immediately or as soon as possible as changes in scheduling do occur.
- Contributes to team effort by performing other related duties as assigned or required.
REPORTING: This position reports to the Program Director or their designee.
