PRIMARY FUNCTION:
To assist patients and families to identify social, emotional, and environmental needs and to connect them with available resources/ services through the provision of a full spectrum of discharge planning and social work services.
EDUCATION AND EXPERIENCE REQUIREMENTS:
- Master's degree in Social Work (MSW) or Master of Social Services degree (MSS) from an accredited graduate school of Social Work.
- Licensed CSW or MSW as required by the state of Delaware no later than June 11, 2024.
- Acute Care Social Work experience preferred.
SPECIAL REQUIREMENTS:
CCM required within 18 months of eligibility
