Job Description
CHEER, Inc is looking for an Outreach Worker in the Roxana and Ocean View, DE area! The primary focus of this position is to coordinate the home delivered meal service at assigned location(s). This includes determining client eligibility, creating meal schedules, and scheduling volunteers.
Location: Roxana and Ocean View, DE
Duties of this position include:
• Receive / intake referrals for Homebound Assistant.
• Conduct home visit / telephone calls to complete client application and assess applicant eligibility.
• Assess client needs for additional services and refer to appropriate departments / organizations. Follow up as needed. Refer ineligible clients to CHEER congregate services or other nutritional / food assistance programs.
• Visit / contact homebound meal clients at least every six months to reassess eligibility.
• Conduct Nutrition Screening on an annual basis for all homebound meal clients and record in ServTracker.
• Creates, coordinates, and communicates meal schedule and changes with Center Directors, Kitchen Managers and Volunteers. Determine appropriate delivery route, sequence, and delivery days.
• Review delivered meal service units daily / weekly with Center Director prior to Center Director posting units.
• Responsible for, but work with Center Director(s), orienting and scheduling volunteers for homebound meal delivery. Maintain Volunteer calendar / scheduler in ServTracker with regular and substitute delivery persons. Verify Volunteer calendar / scheduler accuracy to Homebound Assistant for accurate posting by Homebound Assistant of volunteer service units.
• Deliver meals as needed when Volunteers are not available.
• Community outreach to increase awareness of CHEER services, locate clients in need of service. May include personal meetings and appearances, phone calls, letters, flyer, or other methods as needed.
• Responsible for increasing number of homebound meal clients served.
• Compile, prepare, and submit reports as required by Program as needed.
• Responsible for maintaining assigned Agency vehicle including cleaning interior and exterior on a regular basis and conducting regular maintenance checks.
• Contributes to team effort by performing other related duties as assigned or required.
• This is a Full-time, non-exempt position, hours of operation are Monday-Friday 8:00am-4:30pm.
