ChristianaCare is searching for a full time Community Health Worker to join the Community Health Team to support Silver Lake Elementary School in Middletown, DE. This position will promote access to services, provide health education and educate staff and community members in navigating the health care and social services systems. The team this position is joining is a close-knit work family that values teamwork and supporting each other where needed.
**This position is grant funded; continuation of this position is contingent upon the grant being renewed.
Key Responsibilities:
Attends training provided by ChristianaCare.
Provides peer support as needed.
Develops and delivers health education to patients, students, and the community.
Creates connections between vulnerable populations, the health care system and community/state resources.
Participates in team meetings with School Based Health Center team and Community Health Workers.
Assists patients in the school, community and/or clinic setting.
Helps patients identify socio-economic issues that affect their overall health and develop health/social management plans and goals.
Facilitates healthcare and social service system navigation.
Establishes and maintains professional relationships with community leaders, faith leaders and others responsible for providing services to the community.
Participates in community events promoting The Medical Group and Community Health.
Educates staff, health providers and stakeholders about community needs.
Facilitates educational events and/or small groups for patients, students, or community.
Provides culturally appropriate health education on topics related to chronic disease prevention.
Advocates for underserved individuals to receive appropriate services. Works to reduce cultural and socio-economic barriers between clients and institutions.
Collects data as required by manager
Works collaboratively and effectively within a team.
Establishes positive, supportive relationships with participants and provides feedback to other members of the team.
Builds and maintains positive working relationships with staff, clients, providers, nurse case managers, agency representatives, supervisors and office staff, from diverse cultural and socio-economic backgrounds.
Work Schedule:
Flexibility with schedule is required as this position is based off of the school day and will also require some evening hours and weekends when families are available.
***Also, this position will require fingerprinting during the post-offer, onboarding process.
Education and Experience Requirements:
High School diploma required; Bachelor's Degree preferred
One year community and patient experience required. One year of public speaking, and group facilitation. Excellent writing skills preferred.
Computer experience required. Demonstrate ability to work with diverse populations required.
Bi-lingual language skills are a plus.
An equivalent combination of education and experience may be substituted.
Special Requirements:
Access to personal vehicle.
Ability to work flexible schedule, including some nights/weekends as needed.
Knowledge of New Castle County.
