Job Description
ChristianaCare is currently seeking a full-time Registered Nurse to work as a Nursing Supervisor for the Holloway Community Program. The home base for this position will be located at the Gateway Building on the Wilmington campus in DE.
This position will support all three Holloway Program locations including Smyrna, Georgetown, and Wilmington and will require some travel to these offices.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Administrative Duties
Assists Operations leadership with interviews, selection, and orients qualified clinical candidates.
Orients new clinical staff to clinical procedures.
Supports onboarding and continuing competency assessments of Nurses & Medical Assistants.
Assures compliance with organizational, quality and safety regulatory requirements.
Collaborates with other clinical leadership in developing and implementing performance improvement initiatives.
Performs quarterly-check ins.
Participates in following up on patient/family/physician concerns.
Effectively addresses and intervenes, as indicated, to address and resolve difficult, stressful, or volatile patient situations.
Performs chart audits or has designee assist with chart audits.
Randomly shadows clinical caregivers to ensure appropriate care is delivered.
Supports the site by acting as the liaison for JCAHO, POCT, Tracer monitor for Safe Practice Behavior Monitoring (SPBM) tools & JCAHO Tracers.
Supports practice projects.
Mentors and acts as a resource for registered nurses to encourage development of their interdisciplinary approach and delegation skills.
Collaborates with operation leadership team to provide an organized and efficient office workflow in the practice.
Collaborates with clinical leadership team and caregivers to provide an organized and efficient office workflow
Supports management of day to day operations of the registered nurse that include time keeping, (PTO request and scheduling) coaching, training and adherence to guidelines, policies and procedures.
Assists with daily provider/clinical staffing coverage throughout the day.
Travels to all Holloway Community Program locations.
Clinical Duties
Performs clinical nursing duties including but not limited to taking vital signs, administration of medication, point of care testing, triage, EKG, patient education, appointment scheduling, documenting patient interactions in patient's medical record and other clinical tasks as directed by clinicians.
Performs Transitional Care Management Interactive calls which include medication reconciliation, plans for follow-up visit and provides or arranges for patient education regarding specific health care skills and general disease concepts.
Utilizes unique knowledge, skills, and competencies required to promote wellness, restore health, and support end of life care to assist perform billable services such as Annual Wellness, Advanced Care Planning and Chronic Care Management.
Performs document assessment of more complex patients in office and on the phone.
Demonstrates and integrates cultural differences into patient and family assessment and care planning.
Uses innovative and/or multidisciplinary strategies in ambulatory care planning to achieve desired clinical outcomes.
Performs assigned work safely, adhering to established departmental safety rules and practices.
Reports to Practice Manager and Manager of Nursing services, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.
Performs other related duties as required.
EDUCATION AND EXPERIENCE REQUIREMENTS:
Bachelor's degree in Nursing
3 years professional nursing experience
2 years' experience as a RN, physician office experience preferred
1 year clinical supervisory experience preferred.
SPECIAL REQUIREMENTS:
Current RN licensure in the State of Delaware
BLS certification
professional certification
