Job Description
Are you a NP or PA that is ready to be a part of the future of healthcare? ChristianaCare is currently recruiting for a NP/PA to join our Hospital at Home Team. The Virtual APC Hospitalist is responsible for providing high-quality patient care to various patient populations within the framework of hospital at home model. The Hospital at Home team includes Physicians, Patient Digital Ambassadors, and Virtual Nurses. Our Command Center location is at Avenue North in Wilmington, DE. The selected candidate will work primarily 8-5pm with weekend coverage and the potential of evening shift coverage. They will be required to work from our Command center and home visits.
The selected candidate will serve as the point of contact representing the home hospital program to the patients and family in person, collaborating with the attending physician in the Mission Control and the patient's primary care physician (PCP). They will perform the initial face-to-face evaluation and assessment of potential patient admissions, and upon admission, the NP will collaborate with the attending physician and the PCP to develop and implement an acute plan of care in the home.

CLINICAL DUTIES:
Facilitates the triaging of eligible, appropriate patients referred for a potential admission facilitated by guidelines; obtaining additional diagnostic and specialty consultation when needed
Provides inpatient level acute care in the home setting in collaboration with the home hospital attending physician and Mission Control RN
Helps to support cross-coverage CCHP responsibilities, virtually.
Completes the patient referral and provides safe handoffs to the attending physician and Mission Control RN
Communicates with the patient and family concerning the home hospital model, incorporating patient and family values and goals in the plan of care
Navigates patients and families across the continuum of the phases of acute care.
Navigates barriers for patients and those within the system to accomplish optimal patient experience and outcomes
Promotes a collaborative relationship and effort between the attending physicians, Mission Control RN, and supplier providers
Serves as a role model for professional nursing practice
Maintains and practices in accordance with nursing guidelines per state and hospital regulations
Utilizes advanced nursing knowledge to identify, prevent or solve complex and/or recurring patient care problems
Provides consultation to both nurses and physicians and serves as a resource to staff in area(s) of expertise
Promotes professional practice and a culture of safety; demonstrates willingness to engage in process improvement efforts
Assist with patient acquisition for the HCH team
Assist the boost team with all its activities including transitions of care
EDUCATION AND EXPERIENCE REQUIREMENTS:
Master's degree in Nursing or PA studies
Acute Care NP or FNP certification required
License or license eligible in the state of Delaware
ACLS and BLS required
At least 2 years' experience as a provider preferred
