ChristianaCare is looking to hire a Behavior Health Therapist/ Licensed Clinical Social Worker for one of their growing programs in New Castle, DE!

Christiana Care is looking for an individual that has passion to care for highly complex patients with underlying behavioral health conditions, and who is looking to work in a multi-disciplinary team setting. This role would be serving across three of our practice locations including, Wilmington Hospital-Gateway, Riverside Primary Care, and the Hope Center.

Our intensive program call Center for Hope and Healing is designed for the individual who struggles with a behavioral health condition and in addition, may have suboptimal physical and social health. In a welcoming environment, the program offers multidisciplinary treatment, including psychiatric and behavior therapy care, chronic condition and general health treatment, and comprehensive social health evaluation and intervention. . The intention is to provide flexible, responsive care in a respectful, caring, and supportive setting.

PRIMARY FUNCTION:
To contribute to the overall clinical operation of the Center for Hope and Healing practices and the daily care of the outpatient population in primary care. To function as a member of the clinical team through collaboration, providing consultation, assessment, treatment and behavioral health education.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
Demonstrates the knowledge, skills and abilities necessary to provide psychological care appropriate for the patient population. This requires the ability to conduct assessments, obtain and interpret information, to identify age and culturally-specific patient needs, and to provide the care needed.
Collaborates with interdisciplinary team and community partners as appropriate
Develops group support and therapy activity as appropriate for the specific Medical or Specialty population, and as needed within ChristianaCare.
Contributes to the development of staff by providing consultation, education and suggesting best practice alternatives as appropriate.
Demonstrates responsibility in maintaining established productivity levels.
Demonstrates knowledge of patient's rights and maintains confidentiality of departmental and patient information.
Actively involved in identifying opportunities and implementing strategies to improve performance that impact the patient population.
Attends and participates in collaborative, interdisciplinary rounds.
Develops and provides patient education as appropriate.
Provides virtual coverage for any BHC absent due to sickness, vacation, conference, or leave of absence
Expected to attend face to face or virtual meetings with staff at locations served for networking and problem-solving purposes when appropriate

KNOWLEDGE, SKILL AND ABILITY REQUIREMENTS:
Knowledge of behavioral health principles including assessment, treatment, and diagnosis
Knowledge and ability to conduct brief consultative and formal therapies
Ability to address and prioritize multiple tasks and assignments
Skill in written and oral communication
Knowledge of patients' rights, ethics, and legal issues

EDUCATION AND EXPERIENCE REQUIREMENTS:
Licensed Clinical Social Worker (LCSW), or Licensed Psychologist
Licensed or License Eligible within 30 days
Minimum one year of experience providing direct therapeutic care
