How Will You Make An Impact

RHD is seeking a full-time, Clinician to provide support, direction, and therapy to our consumers on our Intensive Case Management Team In Sussex and Kent county Delaware. This mobile treatment team provides community-based mental health treatment to individuals with serious mental health issues and/or substance. This dynamic team consists of master’s level mental health clinicians, registered nurse, substance abuse specialist, psychiatrist/ARNP, peer support specialist, and a program assistant.

Responsibilities/Duties

+ Acts as part of a multidisciplinary team designed to provide services from a holistic, trauma informed, clinical and values-based perspective.

+ Leads the team of multi-discipline professionals in the development and recertification of treatment plans, goal setting, and overall treatment objectives for each client on case load.

+ Responsible for organizing the input from therapists, case managers, funder assigned advocate, program specialists, behavior specialists, direct support staff, program management, medical assessments, psychological assessments and any other input into a treatment plan with measurable goals, objectives and outcomes.

+ Provides support to program management in assessing crisis intervention when needed from a trauma informed perspective.

+ Conducts screening and assessments using validated screening tools to assess for depression, trauma, substance use, and risk/needs associated with criminal risk.

+ Completes all assessments and documentation as required by RHD and others to support current and ongoing services.

+ Participates in quality improvement and training activities for the purpose of compliance and program excellence.

+ Other duties as assigned.

Required Experience

+ Prefer 1 year of experience, but willing to work with the right individual to join our team.

Required Education

+ Master’s degree in Social Work, Counseling, Psychology, or related field

+ Master’s degree in Social Work/ Psychology
