POSITION SUMMARY: Responsible for health care assessment, care and documentation, dispensing medication, and providing direct client care to Elwyn participants.


Assist the Healthcare Supervisor to assure compliance with current program policy and procedures governing the provision of medical services to the participants and with all state and federal regulations regarding standard nursing practices

Implement all applicable policies and procedures to assure that each participant receives his/her medication(s) as prescribed by the physician and that administration of medication(s) is documented in an appropriate manner

Provide and monitor direct client care and other activities as required and as ordered by the physician including but not limited to dietary requirements, mobility assistance and wound care

Provide care to participants within applicable programmatic and licensing standards

Recommend best practices to the team in response to observation of healthcare needs

Conduct routine contact with participant's family and doctor in order to obtain changes in medication and/or to obtain or provide information about the participant's health status

Instruct and observe participants in self-help and self-care activities so as to maintain as high a level of independence as possible with personal hygiene, grooming, nutrition, and community living

Respond directly to medical emergencies, provide emergency treatment, and implement emergency medical procedures as documented in the program policies and standards of care for LPNs

Assist in the development of individual care plans for all participants and assist the Support Specialist to maintain the care planning system

Document observations, assessments, and interviews with participants in the medical record as indicated

Request and maintain adequate medical and first aid supplies for program

Assist as part of the team to instruct participants in the areas of recreational therapy, reality orientation, socialization, and daily living activities (i.e. walking, eating, toileting, grooming, and supervision)

Report progress and health/social status of all participants and family doctor contacts to the Program Supervisor

Identify participants' day to day health care needs, and in conjunction with the Healthcare Supervisor, coordinate with health agencies to meet those needs

Monitor and record vital signs as necessary including but not limited to monthly blood pressure and weights

Monitor nutrition on all participants; report dietary changes, other related issues and problems to the Program Supervisor

Maintain an adequate supply of participants' medications through requests to the family or residential provider

Assure an effective mechanism for ordering newly prescribed medications, changing labels as needed, and other related medication tasks as needed for participants who need additional support to get their medications to the program

Supervise and assist participants who wear hearing aids

Assist Support Specialist with the referral/intake process by completing participant nursing assessments

Assume responsibility for daily healthcare operations in the absence of the Healthcare Supervisor

Function as the "on-call" LPN for Elwyn Delaware responding to medical emergencies and first aid situations in accordance with standard nursing practices and as documented in the program policies

Administer medication to individuals in other Elwyn Delaware program(s) who are unable to self-medicate and provide appropriate documentation as indicated in the program policies

Perform cardiopulmonary resuscitation (CPR), crisis intervention and medication administration using agency-trained protocols

Perform other related duties as assigned
