How You’ll Make an Impact The Case Manager will provide support, direction, and overall support to our Intensive Case Management Team in Sussex and Kent County Delaware This mobile treatment team provides community-based mental health treatment to individuals with serious mental health issues and/or substance This dynamic team consists of master’s level mental health clinicians, registered nurse, substance abuse specialist, vocational specialist, psychiatrist/ARNP, peer support specialist, and a program assistant. Investment in Growth: We offer programs and workshops to our staff that support furthering education and the development of leadership skills ****APPLY TODAY TO JOIN OUR TEAM*!!!* *Responsibilities/Duties Responsibilities/Duties * Act as part of a multidisciplinary team designed to provide services from a holistic and Values?based perspective.? * Assist in the development of recovery plans and reviews in collaboration with the program participant, clinical and medical staff and other stakeholders as determined in collaboration with program participant. * Provide community-based services to program participant focusing on education, connection with resources, community interaction and achievement of recovery plan goals.? * Complete all assessments and documentation as required by RHD, funder, licensing/ accreditation entities.? * Develop and maintain relationships with community agencies in order to assist program participants in connecting with all available resources.? * Other duties as assigned Required Experience * 3 years of relevant experience in mental health and drug and alcohol Required Education * Bachelor’s degree in Social Work, Counseling, Psychology, or Rehabilitation Required Qualifications Bachelors Degree in related field 3 years of relevant experience Program Summary Sussex Intensive Case Management Team is a community-based mobile program that works with people to encourage independence and empowerment them to succeed every day.
