Job Description

n an effort to improve health outcomes in a specific target population, the CHW works with community based organizations (CBOs) and their members to address social determinants of health (SDoH). The CHW assists in the delivery of culturally appropriate and community-based education for the specific target population.   S/he collaborates with community based organizations to engage community members, conduct social determinants of health assessments and make appropriate referrals in order to meet persons identified needs. The CHW will work with other assigned team members and organizations to provide direct, logistical, and administrative support for target populations.


Role and Responsibilities:
• Will assist with community outreach events both in person and virtually at engaged Community Based Organizations (CBOs).
• Will engage female targeted community members in well woman discussions and screenings for SDoH.
• Reviews results and makes referrals to resources to address identified SDoH. May follow up with patients and referral agencies to assure services were rendered. Follows up with referral organizations on member engagement Leads well woman lifestyle and serves as a positive role model within communities served
• Motivates members of the community to be an active engaged participant in their health
• Travels for outreach events. Flexibility in schedule and ability to work some evenings and weekends as needed.
• Takes primary responsibility for all data collection and entry required for internal project processes.
• Provides technical assistance to others internal and external to the organization in using and evaluating the effectiveness of specific intervention approaches.
• Other duties as assigned.

• Ability to work collaboratively in team environment, maintaining effective working relationships with all team members, including staff throughout Quality Insights.
• Ability to effectively communicate the importance of wellness and self-care at all stages of life
• Has good verbal communication skills and an engaging disposition especially with the targeted populations
• Ability to comfortably interact with diverse populations
• Ability to be compassionate and non-judgmental with community members
• Available to travel throughout Delaware 
• Reliable transportation is required
• Working knowledge of community resources in Delaware
• Ability to organize, support, and implement project activities.
• Ability to work with broad array community-based organizations. Ability to work independently, knowing how to seek and use supervision while carrying out a range of complex projects.
• Ability to recognize when to improve needed skills through research, supervision, formal training; has to be a quick and motivated learner.
• Accurate and complete project documentation in database
• Must have operational computer skills, with moderate knowledge of wide range of productivity suite applications and ability to learn new applications in a timely manner.
