POSITION SUMMARY: Provides direct supervision to Journeys program staff and is responsible for day to day operations, work assignments and program compliance oversight.
DUTIES AND RESPONSIBILITIES:
Provides oversight of program operations including but not limited to:
case management services including individual program service plans
advocacy for individuals served and their caregivers
monitoring the coordination of all services and safety protocols provided to the individuals served
creation and implementation of activities and schedules
regulatory compliance, including but not limited to staff training and daily program activities
Provides supervision of assigned staff in the areas including, but not limited to, recruitment, selection, leadership, coaching, training, performance management, promotion, discipline and termination while challenging staff to continually seek quality improvement in service implementation and giving encouragement and recognition for a job well done
Facilitates admissions process which includes, but is not limited to, scheduling and conducting tours, intake interviews, making recommendations concerning acceptance, and obtaining service authorizations
Provides direct care services, when necessary, including but not limited to running activities with participants and providing personal care such as assistance with toileting, eating, and brushing teeth
Provides work direction to assigned staff which includes but is not limited to monitoring quality of work; ensuring that participants receive appropriate, quality services according to their individual service plans (ISPs); ensuring that active treatment is occurring at all times; making and adjusting caseload assignments; scheduling and assigning staff to daily activities
Facilitates the daily morning huddles and weekly staff meetings, including but not limited to sharing company and policy updates, promoting cross communication among staff and encouraging the use of best practices; communicating pertinent participant related information, and assigning daily tasks
Coordinates case records review by case managers to ensure file compliance with regulatory guidelines
Monitors daily service notes, T-logs, electronic medical record entries, and attendance records, as well as the weekly, monthly, quarterly and annual reports completed by the case manager to ensure they are completed accurately and submitted timely
Develops the monthly activities calendar in collaboration with assigned staff, and assign daily activities to staff as scheduled
Ensures that supplies are adequate for implementation of daily activities by completing visual inventory and submitting purchase order requests to Director of Operations
Researches and coordinates community outings and volunteer sites for inclusion on activities calendar
Obtains permission slips and fees for scheduled outings, record receipt, and give all payments to appropriate Finance Department representative
Performs regular audits and provides guidance and support to the Case Manager to bring records into full compliance
Supervises and performs, as needed, the duties of the program case manager, which includes but is not limited to, completing the following services for individuals on his/her caseload: acts as a liaison with interdepartmental staff, outside agencies, families, etc.; coordinates the completion of individual assessments; promotes the development of social and vocational development of individuals served; develops the individual's ISP. This may include participation in off-site meetings.
Works with the director to meet the total needs of the program which includes but is not limited to development and implementation of outcome measures (assures the ISPs are well developed, are appropriate and respect the individuals preferences, are coordinated with other services provided, are integrated into the person's life, and reviewed in a timely manner); and assurance of the efficient use of shared program resources
Provides program coverage for Case Manager and Day Habilitation Associates as necessary which may include coverage of volunteer and activity sites in the community
Oversees management of critical incidents, which would include but is not limited to, reporting, recording, and following up on critical incidents as outlined in Critical Incident Protocol, workers comp incidents, allegations of abuse or neglect and oversight of daily operations
