Summary LPN Specialty Clinic position is located at the Wilmington VA Medical Center, and is accountable to Nurse Manager, Specialty Clinic; Associate Chief Nurse Acute Care and Specialty Clinics; Associate Director Patient Care Services Responsibilities The LPN will initiate appropriate actions based upon the plan of care and his/her observations of the patients’ condition under the supervision of the registered nurse and/or physician and utilizes independent critical thinking. He/s he can vary the order and sequence of assigned work and use judgment in selecting most appropriate actions. Completed work needs only a general review for technical soundness, appropriateness and conformity with policy and nursing assignments. Performs complex practical nursing care with precision and efficiency, consistent with Medical Center policy and procedure and within the legal and ethical responsibilities of his/her licensure as a Licensed Practical Nurse. Performs specialized practical nursing activities requiring broad clinical experience and demonstrated skills sufficient to resolve a range of nursing problems. Provides safe and individualized nursing care to patients with changing complex physiological and/or emotional needs. He/she will take into consideration the patient’s age related abilities associated with mobility, nutrition, mentation, hygiene, continence, psychosocial status and cultural needs. Will act urgently if needed and report changes in patient’s condition and other significant observations to an appropriate team member in a timely manner and documents appropriately in the medical record. Maintains confidentiality. Assists with patient and family/significant other teaching under the supervision of the registered nurse. Identifies learning needs, suggests ways to meet identified needs, implements and evaluates the teaching plan in conjunction with other staff. Fully participates in the clinic process. Gathers data as assigned by the RN and communicates this data verbally and/or in writing according to established procedures. Performs a full range of complex treatments and procedures, ordered treatments and procedures, according to Medical Center policy/procedures, e.g. uroflow studies, EKG, phlebotomy, administration of immunizations, and other medications as ordered by providers. Reviews and completes medications orders. Prepares, administers and documents medications with an understanding to medication actions, side effects, adverse reactions and contraindication following Medical Center policies/procedures. Performs intradermal tuberculin testing. Establishes constructive interpersonal relationships with patients and families/significant others. Establishes professional relationships with members of other disciplines and with support personnel who provide patient care. Uses effective communication skills. Uses knowledge of human behavior, motivations and reactions to events in dealing with others in difficult situations and promotes positive relationships, communication and socialization skills. Works to establish team building. Recognizes and responds appropriately in emergency situations according to established center procedures; for example, codes, fire, disaster, hazardous spills, rapid response and abuse. Fully participates as a member of the specialty clinic team providing patient care. Contributes to the specialty department in promoting patient wellness through education and referrals and encouraging patient participation in the therapeutic community by fostering self-reliance and independence. Uses the computer to access patient information and enters patient information accurately and appropriately. Participates in work groups, teams and /or committees and supports the Center QM/QI activities and organizational goals. Collects data to support any unit PI projects. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Written and Spoken Physical Standards: All applicants must meet the physical standards for the position. A physical examination prior to employment may be required. Licensure: Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia. Education: Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC)] at the time the program was completed by the applicant. Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience. Preferred Experience: 2+ years recent nursing experience; preferably outpatient setting BLS certification Grade Determinations: In addition to the basic requirements stated in section A, the following qualification criteria must be met for each grade. The candidate’s qualifications must clearly demonstrate the level of competence required for the grade. GS-3 None beyond the basic requirements. GS-4 Six months of qualifying experience as an LPN or LVN; or Graduation from an approved school and one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures; or Graduation from an approved school of at least 24 months duration. GS-5 Completion of at least 1 year of qualifying experience at the GS-4 level or equivalent. Demonstrated knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems. Works with the RN and/or MD/DO to appropriately orient/train less experienced LPNs/LVNs and/or Nursing Assistants (NAs)/Health Technicians (HTs) in relation to acceptable standards of practice in promoting optimal patient care delivery. Provides effective education to patients and/or family members in relation to common disease processes, medication, and /or prescribed treatment regimes. Demonstrated ability to serve as a responsible member of the nursing team and interact in an appropriate manner with patients, family members, professional and other supportive personnel involved in the delivery of patient care, incorporating acceptable, established customer service standards into practice.
