As a Part Time 20 hours a week Nurse Liaison for ChristianaCare HomeHealth you will identify home care needs of patients to ensure a smooth transition from acute care and post acute care settings. The Nurse Liaison supports the intake process by interacting with discharge planners, case managers and patients within the acute and post acute care settings. The successful candidate will work out of our Camden, DE office.
RN preferable, though LPN's may apply. RN must either possess a BSN or be willing to acquire within three years of start date.
Knowledge of medical terminology, coordination of services and Home Health practices.
Strong interpersonal and customer service skills
Excellent communication and organization skills
Ability to work and travel to hospital and post acute care settings
