Job Description

ChristianaCare is searching for a Health Educator for the Women's Health Services Department to promote access to services, provide health education and assist patients in navigating the health care and social services systems.

 

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Develops and delivers health education for Child Development Watch patients and families.

Creates connections between vulnerable populations, the health care system and community/state resources.

Assists patients at ChristianaCare Child Development Watch.

Helps patients identify issues affecting their overall health and develop health/social management plans and goals.

Facilitates healthcare and social service system navigation.

Educates staff, health providers and stakeholders about patient needs.

Provides culturally appropriate health education on topics related to chronic disease prevention.

Advocates for underserved individuals to receive appropriate services.

Collects data and works with leadership in preparing reports.

Maintains professional competence by participating in local and national training sessions and conferences.

Works collaboratively and effectively within a team.

Establishes positive, supportive relationships with participants and provides feedback to other members of the Child Development Watch team.

Builds and maintains positive working relationships with the patients, providers, agency representatives, supervisors and office staff, from diverse cultural and socio-economic backgrounds.

Participates in regular program and department meetings.

Performs assigned work safely, adhering to established departmental safety rules and practices; report to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.

Perform other related duties as assigned and required.

 

EDUCATION AND EXPERIENCE REQUIREMENTS:

Bachelor's degree preferred.

 

One-year community and patient experience preferred.

 

Computer experience required.

 

Demonstrated ability to work with diverse populations required.

 

Bi-lingual language skills are preferred.

 

An equivalent combination of education and experience may be substituted.
