Description

As a Veterinary Patient Care Coordinator you will:

+ Take primary care veterinarian referral calls when the doctor is not available for transfers, follow up question or consults.

+ Attends rounds in the morning to learn about any cases being transferred to the specialist’s service, ascertains what the plan for the pet was for the day and once instructed by the specialist, play the role of liaison between the specialist and client.

+ Works closely with all departments in the hospital to improve workflow functionality in the hospital geared towards patient care and communication efforts.

+ Schedules necessary department meeting to verify that all resources are being utilized appropriately and new processes are implemented into the PCC responsibilities.

+ Orients and educates pet owners and their families by being available to them via phone, email or in person, explaining the role of the patient care coordinator; providing educational information regarding treatments, procedures, medications, and continuing care requirements.

+ Coordinates information and care requirements for the pet; resolving issues that could affect smooth care progression; fostering team support for the pets care and treatment.

+ Aids in assuring follow-up call backs are performed for all patients discharged from a specialty service. Assures clients have a full understanding of the expectations of recovery, and are provided reassurance that Bluepearl is still heavily involved in the care of our patients after being discharged.

+ Takes calls from pet owners of pets not in hospital who have prescription refill requests, concern for a change of status of the pet, food inquires and any other issue the pet owner is concerned about regarding their pet.

+ Adapts to the specialists style and instruction recognizing that each specialists has his/her own desired method and style of communication

+ Documenting conversations with specialist and clients in the medical record; interacting with other services to expedite scheduling, procedures, and consults.

+ Respects patients and owners by recognizing their rights; maintaining confidentiality.

+ Maintains quality service by establishing and enforcing organization standards.
