Job Description

PRIMARY FUNCTION
The care management intern works under the direction of a Center for Virtual Health RN providing supportive care management functions, including, but not limited to, patient education, payer authorizations, post-discharge and/or follow-up phone calls to patients to ensure timely delivery of equipment and services, completion of health questionnaires with patients, and entry of quality measure data into software systems.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
Provides patient education to members, patients and families as directed by the primary Center for Virtual Health RN. Alerts Center for Virtual Health RN to necessary follow-up.
As directed by the Center for Virtual Health RN, ensures pertinent elements of the care management care plan are addressed timely.
Assists in the enrollment of patients and beneficiaries in a care management program according to established program guidelines
Provides patients, beneficiaries and caregivers with resources to reduce barriers to accessing care and increasing engagement with healthcare providers.
Identifies problems or gaps in community resources that impact outcomes, care or utilization of services and communicates problems or gaps to the Center for Virtual Health RN.
Assists Center for Virtual Health RNs with closing gaps in care for HEDIS quality measures.
. Enters timely and accurate data into Care Management applications necessary to communicate patient needs and to ensure complete documentation of patient needs and phone calls
Facilitates access to entitlement programs and/or community resources
Adheres to organizational and departmental policies and procedures and credentialed compliance
Demonstrates proficient PC skills in a Windows based environment including word processing, spread sheets and working in database programs
Performs all other related duties as assigned by Clinical staff, Supervisor, Manager or Director.
SCOPE AND FREQUENCY OF CONTACTS:

Ongoing contact with patients, beneficiaries, caregivers, multidisciplinary care teams, eBrightHealth ACO leadership and staff, Carelink staff, insurance /managed care organizations, community resources, home health care providers, durable medical equipment providers, external and internal customers.

Center for Virtual Health RN - provides daily assignment
Supervisor, Care Management
Manager, Care Management

EDUCATION AND EXPERIENCE REQUIREMENTS:
Must be actively enrolled in a university-based health sciences degree program and be approved by the appropriate university faculty for this internship.
Must maintain established academic performance per university guidelines
Knowledge of medical terminology
Proficiency working with computers and various computer software applications.

KNOWLEDGE, SKILL, AND ABILITY REQUIREMENTS:

Strong organization skills
Excellent customer service and interpersonal skills on phone and in-person
Ability to handle multiple tasks simultaneously
Ability to maintains confidentiality of all company, patient, and beneficiary information
