Job Title: Nutritional Consultant
Location: Remote / Work From Home
Schedule: Mondays through Fridays, 9 AM to 530 PM EST
Responsible for providing nutritional/dietary consultations to participants in the patient management programs.
Primary duties may include, but are not limited to:
Ensures patient understanding of nutritional goals and objectives; provides nutritional counseling and patient education for patients diagnosed with chronic conditions.
Motivates patients to make changes or enhancements to their current nutritional plan.
Collaborates with physicians or other health professionals to ensure adherence to prescribed plan of care.
Develops training modules for nurse consultants providing care management services for participants.
Requires an RD and minimum of 3 years of experience counseling individuals regarding nutritional/dietary management issues related to chronic disease; or any combination of education and experience, which would provide an equivalent background.
Proficient in Microsoft Windows and Microsoft Office (Outlook, Word, Excel, PowerPoint) highly preferred
Strong research and analytical skills as well as communication skills (both written and verbal)
Ability to document/type while counseling and viewing clinical or other references
Ability to multi-task is essential
Experience with direct patient counseling or education highly preferred
Call center experience and/or telephonic counseling/coaching preferred
Weight management and /or CDCES Certification is a plus
Fluent and bilingual in Spanish and English is a plus
Knowledge of managed care and integrated health systems preferred
For candidates working in person or remotely in the below location(s), the salary range for this specific position is $50,688 to $101,376**
Locations: Colorado; Nevada, Jersey City, NJ; New York City, NY; Ithica, NY and Westchester County, NY
