$4000.00 Sign-on Bonus- 50% at day 90; 50% at day 180 after successful completion of the initial observation period***
$4000.00 Retention Bonus- at the completion of 12 months of successful employment***
$40.05 per hour rate for a Group Home Program RN
Typical work schedule is 8AM- 4PM M-F (some exceptions apply)
The Registered Nurse (RN) position works closely with designated care teams which may include Counselors, Therapists, Case Managers, Direct Support Professionals, Physicians/Psychiatrists and program administrators to plan and implement care, including patient education, care management, triage, medication reconciliation and other duties. A commitment to the RHD values should be demonstrated as job duties are performed.
+ Administer and/or oversee the administration of medication as prescribed by the physician of record to all consumers within the program(s) to which they are assigned.
+ Obtain accurate vital statistics (height, weight, temperature, blood pressure, pulse, respirations, etc.) for purposes of effective treatment as needed per program parameters.
+ Assess and respond to new/potential/ ongoing health issues in persons served.
+ Assess mental status and report to physicians
+ Coordinate and schedule all medical (annual, emergent and follow-up care)
+ Escort consumers to all medical/psychiatric appointments (including telehealth)
+ Act as part of a multidisciplinary team designed to provide services from a holistic and Values based perspective
+ Manage the ordering, delivery, and storage of medications for the people served in accordance with the specific program or service
+ Manage and maintain the Medication Administration Record for accuracy
+ Complete documentation as required by RHD, funder/licensing/accreditation bodies
+ Order and stock medical related supplies for house and consumers
Training and Development
+ Provide education to both people served and staff regarding common health concerns.
+ Provide training to staff as required/requested in order to best meet the needs of people served.
+ Become a State of Delaware Certified LLAM trainer and conduct regular trainings with staff
+ All other duties as assigned
+ 1-2 years of residential/long-term care experience preferred.
+ 1-2 years working in an environment that supports those with mental illness or developmental disabilities.
+ A graduate of a school of professional Nursing (AD, hospital, or BSN); BSN preferred.
+ Current license to practice in the state of operation
+ Basic computer and internet literacy with the ability to utilize applications like emails, time recording and other programs to enter and retrieve data as necessary to perform the essential functions of the job.
+ Valid Driver’s License
+ Must Pass Background
+ Must Pass Drug Screening
+ CPR and/or BLS certified preferred
+ Two-step TB Test required by State of Delaware Long-Term Care
+ If not Fully vaccinated for COVID you will be required to provide Weekly COVID test results to be work eligible.
+ Must be able to use a computer and provide clinical documentation of services.
+ Medium Physical Requirements: exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds constantly to move objects.
Grubb Rd Passages is a group home that services women experiencing challenges in their mental health that impedes independence in the community. The home services up to 5 individuals. Currently the home is located in Wilmington, yet we are moving to Newark, DE in mid-Spring. Services provided here focus on skill building, symptoms management, activities of daily living as well as community living to maximize their quality of life and reducing the need for impatient hospitalization.
