ChristianaCare is looking for an energetic, organized leader with experience in community engagement to oversee the day-to-day operations of Community Health programming, specifically Project Equinox High Performing Social Care Network. This role requires strong project management and organization skills. The ability to facilitate meetings, create and maintain relationships and make community presentations is also paramount to the success of this role. Some responsibilities include:
Attends training program provided by ChristianaCare and the Penn Medicine Center for Community Health Workers
Successfully work closely with internal and external partners to create, facilitate, and support the High Performing Social Care Network
Responsible for the day-to-day administration and management of Community Health programming, specifically the Project Equinox High Performing Social Care Network
Recruit and supervise Community Health Worker - interviewing, hiring, training and all day-to-day oversight requirements
Assures that program goals are met, and data is collected by staff as required
Desired candidates will hold a High School Diploma (Bachelor's preferred) and have two years' experience in program administration, program leadership, coordination, etc. People leadership and bilingual skills are a plus. An equivalent combination of education and experience may be considered
