Description/Job Summary
The Wellness Coach assures that all operational procedures are followed. The Wellness Coach is responsible for utilizing psychiatric rehabilitation principles to promote recovery, community integration and improved quality of life for residents through collaboration with consumers, team members, family members, significant others and collateral resources. The Wellness Coach will assist team members and other providers with an understanding of the Psychiatric Rehabilitation Model and the day-to-day practice of this modality. Wellness Coach is responsible for working in accordance with the values and policies of RHD.
Responsibilities/Duties
Utilizing psychiatric rehabilitation principles to promote recovery, full community integration and improved quality of life for residents To maintain a safe, supportive and recovery-focused environment that fosters growth and rehabilitation as well as facilitates the development of natural supports and self-help skills To engage consumers in a trusting and supportive relationship, providing emotional support within the professional boundaries, as needed To support the individualized recovery plans developed with consumers with the aim of achieving and maintaining physical health, mental health and emotional well-being as well as participating in the recovery process. To assist the consumer to reach the goals he or she has set in their recovery plan Practice and encourage advocacy activities to enable self-advocacy efforts.Complete required daily documentation and other assigned documentation in EHR (Avatar).
Become LLAM certified and administer medications to the residents. Provide transportation to residents for medical appointments and community outings.
Support consumers in activites of daily living, meal prep, hygiene, laundry, meet goals on residents treatment plans, ect… Complete other duties as assigned by supervisor.
Attend monthly staff meetings
Run groups with residents.
Complete assigned monthly trainings (online via Cornerstone).
Maintain annual TB/PPD, LLAM Training (medication administration), and First Aid CPR certification.
Other duties as Assigned.
Required Experience
Prior experience in Social Services is prefered
Required Education
Bachelors degree in Behavioral Science, Psychology, Sociology, Human Servises, Helath Care Administration, or similar to Mental Helath Field.
Required Qualifications
Valid Drivers licensePrior experience in Mental and Bevaoiral Health Group Home (Preferred)
Covid vaccination or weekly testing.
Excellent written skills
Ability to use electronic devices (including lap tops, computers) to complete EHR paperwork
Experience in working as a team member
Critical thinking
