Description/Job Summary

RHD Brandywine Hills Group Home is a residential program for 8 adults with serious mental illness. Services focus on skill-building in symptom management and community living, maximazing quality of life, and reducing the need for inpatient hospitalizations.

Brandywine is seeking:

+ Full Time LPN to work (Days) 40 hours per week. Shifts available: Monday Friday 8 AM 4 PM with flexibility of weekends if needed.

+ Overall Responsibility: Collaborate with the Assistant Program Director and a multidisciplinary team in providing high-quality health care services to adults in a residential setting.


FT – 3000.00 SIGN ON BONUS AND 3000.00 RETENTION BONUS AVAILABLE***

Responsibilities/Duties

+ Support residents in establishing and maintaining optimal physical and mental health.

+ Assess mental and physical health needs/concerns on a daily basis.

+ Provide counseling as needed to residents on chronic health care issues.

+ Facilitate pspychoeducational groups in support of mental and physical health.

+ Maintain standards for medication documentation,ordering, storage, amd disposal that is in accordance with DSAMH, CARF, LLAM, and RHD regulations and policies.

+ Attend weekly treatment teams to collaborate with pyschiatrist and other providers.

+ Schedule, attend, and follow-up on medical appointments in accordance with DSAMH, CARF, and RHD regulations and policies.

+ Provide education to staff on health issues relevant to the population and residental setting.

+ Communicate with pharmacy, medical personnel, and other community supports on an ongoing basis to assure resident needs are met.

+ Provide support in urgent or crisis situations (in person or telehealth).

Required Experience

1 year experience required working in the mental health field.

Required Education

Associate Degree

Required Qualifications

+ Must be a Delaware Licensed Practical Nurse (Registered).

+ Driver’s License required.

+ 1 year working with mental health population.

+ Ability to multi-task and work cooperatively as part of a multidisciplinary team that includes; CNA, clinicians, psychiatrist, RN’s and LLAM trained personnel.

+ Residental experience preferred.
