The individual in this position will coordinate and provide guidance, counseling and encouragement to participants as they transition from residential services to a more independent environment. The member in this role will implement and document participant progress in accordance with individualized rehabilitation plan. Ensure a safe, healthy, secure, and pleasant environment which promotes growth and development.
- A demonstrated ability to identify participants strengths and to assist participants with learning to benefit from those strengths
