The Certified Nursing Assistant (CNA) is a member of The Moorings at Lewes Health Care team. The CNA is responsible for providing direct care to residents, within the physical and psychosocial realms and in accordance with the Interdisciplinary Plan of Care. The CNA adheres to standards, protocols, policies & procedures, work rules and laws addressing resident care, service quality, job performance, communications and teamwork.
Demonstrated knowledge of residents’ rights and respects and promotes and protects these rights at all times. Receives report on assigned residents at the start of each shift and gives report to nurse at the end of each shift. Makes rounds on all assigned residents at the beginning of the shift and ensures that all residents are seen at least every hour. Provides/assists with physical care to assigned residents including, but not limited to: Mouth Care, Bathing, Skin Care, Grooming including trimming fingernails (excluding diabetics). Dressing, Feeding/Assisting with meals.Toileting including catheter and colostomy care. Turning, Positioning and Restorative care. Ambulation and/or Range of Motion exercises. Bowel and Bladder training. Weighs residents as directed by RN/LPN and accurately records weight. Ensures that residents are well hydrated by offering/encouraging fluid intake with meals, and every 2 hours when awake. Takes and documents Temperatures, Pulses & Respirations, and blood pressures as assigned. Monitors and documents intake and output as assigned. Monitors and documents bowel movements. Monitors and documents other aspects of resident care as assigned. Collects specimens as assigned.
Ensures resident safety by: Maintaining a clean, neat environment. Assisting/dressing resident in appropriate clothing/shoes. Answering call lights and security alarms, promptly. Demonstrating appropriate communication skills and actions in dealing with agitated residents. Assisting residents with transfers/ambulation as needed. Reporting changes in resident behavior/condition to RN/LPN. Reporting changes in resident behavior/condition to RN/LPN. Functioning as an advocate for residents as needed. Assists with resident activities as assigned. Participates as a member of the Interdisciplinary Team. Provides input into the plan of care. Implements assigned aspects of resident plan of care. Assists with admission, transfer or discharge of the resident. Transports residents as needed or as assigned.
