ASSESSMENT COUNSELOR I
New Castle County Probation & Parole (Cherry Lane)
Under limited supervision, conducts client admission into treatment program and completes comprehensive assessments. Develops and implements initial treatment plans for clients with dual diagnoses, social or mental health issues. Serves as professional resource to other staff in resolving complex intake problems. Develops and conducts in-service and continuing education programs for treatment staff.
ESSENTIAL JOB RESPONSIBILITIES
1 Regularly travels to various program sites and centers to complete comprehensive assessments according to program guidelines, and formulates diagnostic impression, by conducting client and/or family interviews, reviewing substance abuse and mental health treatment history and conferring with staff and referral sources. Reports admission activities and progress to supervisor.
2 Determines client’s appropriateness for treatment, level of care, strengths and problems, potential dual diagnosis, and requirements for other ancillary services. May obtain information by telephone to conduct preliminary admission screening.
3 Prepares initial client treatment plan in accordance with assessment and evaluation, including assigning client to appropriate group and counselor; and identifying need for referral resources or services, such as parenting classes, job search support, education, and medical care.
4 Explains treatment plan and objectives, orients client to treatment program and facilities, and explains policies and rules. Provides client with relevant information, such as meeting schedules and locations.
5 Provides basic didactic group counseling, including psychoeducation, drug education, and family education to ensure peer support occurs.
6 Provides individual client counseling, educational programs and lectures to address relapse prevention, problem solving, behavior modification, anger management, etc., and to assist in overcoming substance use disorder, adjusting to pro-social life, and making cognitive and behavioral changes.
7 Develops and maintains regular contact (usually by telephone) with family members and/or significant others; as well as various outside agencies, such as social services, corrections, protective services, probation or parole; in order to facilitate assessment process, and complete necessary treatment follow- up and case management.
8 Performs initial case management to facilitate program admission and retention, including encouraging and motivating clients’ participation and commitment to treatment, directing clients to additional services, contacting various agencies and insurance carriers to resolve admission problems, following up on appointments missed by clients, providing crisis intervention.
9 Collects and maintains treatment records and reports to ensure organizational, regulatory, accreditation, and contractual compliance.
10 Serves as professional resource, and may provide clinical supervision to other Counselors:
- audits client records prepared by others to ensure compliance with required documentation and service delivery
- provides assessments of special needs clients to advise and guide to resolve complex case problems
- demonstrates and/or instructs other staff on appropriate procedures, modalities and techniques
11 Performs other duties as assigned, including conducting dormitory inspections; initiating crises management and conflict resolution interventions; writing disciplinary tickets and incident reports; and notifying corrections staff of clients’ disruptive behaviors, or potential threats to safety of others.
MINIMUM EDUCATION AND EXPERIENCE * Bachelor’s Degree in psychology, social work, mental health counseling, or substance abuse counseling; or equivalent education and life experience, with a minimum of 100 hours documented clinical training in counseling theory and practice * Three (3) year experience in substance abuse treatment, including at least one (1) year experience working with dual-diagnosed clients.
